If you’re not sure if the team is already in the DSU Event Management Database use the “search” field in the linked table “Look Up Team” below which will open in a new browser tab to determine if the team a dog is a member of is in the DSU Event Management Data Base then return to this page.

If the team exists (You find a record for it in the linked table below or know it already exists in the database) skip the form and  proceed directly to step 4 by clicking the “Next” button at the bottom of the page.

If the dog is not a member of a team proceed directly to step 4 by clicking the “Next” button at the bottom of the page.

If the team a dog belongs to is not already in the data base, use this form to add it into the system.  Press the “Add/Update” button to enter the team into the system and then press the “Next” button at the bottom of the page to proceed to step 4.

Proceed to step 4
